A circled take tells the editor what the director liked. This sheet tells them why — and that's the difference between a useful note and a mystery.
The reason and performance columns are where the value is. "Circled, but the overlap kills the front of the line" or "alt select is the better read, weaker end" is worth more to the cutting room than any number of ticks. So is flagging noise and overlap: the editor finds out about a plane or a stepped line eventually, and it's cheaper for everyone if they find out from you rather than from the timeline at 11pm.
