Sound Report Template
Free printable production sound report. Log scene, take, file/roll, boom and lav, room tone, wild tracks, usability and noise issues — US Letter PDF, no signup.
The mixer's record of the day, and post's first line of defence. Take by take: which file, what was on the boom, what was on the lavs, whether it's usable, and what went wrong if it isn't.
The ADR risk note is the one that earns its keep. A take that's fine on headphones but has a fridge hum under it, or a line half-buried by an overlap, is far cheaper to flag now than to discover in the mix. Post reads these notes; write them for the person who wasn't there and can't hear what you heard.
