Meal penalties are money, and money gets argued about. This log keeps the times as they actually happened: when people came in, when the meal was called, when they went back, when they wrapped.
Write the meal rule at the top before the day starts. Whatever the agreement is on your production, the person filling this in needs it in front of them rather than in their head — grace decisions are much easier to defend later when the rule was written down first. The notes column is for exactly those judgement calls, and for anything payroll will need to escalate.
