The cast equivalent of the crew time log: when each performer checked in, when they were on set, meals, travel, wrap — and a signature column, because these times get signed off on the day rather than reconstructed afterwards.
This is a generic Exhibit G-style layout, not an official union form. It's laid out to capture the same information in the same order, which makes it useful for a shoot that needs a clean record or a non-union production that wants one anyway. If you're working under a specific agreement, use whatever form and terms that agreement requires — and treat this as a working record rather than a substitute for it.
