Lost time is only arguable if nobody wrote it down. This sheet exists to record delays while they're happening — start, end, minutes, cause, and which department it sat with — so the day's losses are a record rather than a disagreement at wrap.
Write the reason plainly and without blame; "generator fault, 40 min" is more useful than a narrative. The recovery checklist is the part that changes the outcome: it forces someone to actually confirm the cause with the department head and communicate the new timing, instead of quietly hoping to make it up later in the day.
