Diffusion is the reason this sheet exists. Nobody forgets which lens they were on, but three weeks later nobody can agree whether that scene had a 1/4 Black Pro-Mist on it — and if you get it wrong on the pickup, the two shots will never quite match.
So: focal length, stop, ND, diffusion, any diopter, plus FPS and shutter if either moved off base. VFX will ask for exactly this data along with a lens grid, and the answer is much easier to give if somebody wrote it down on the day rather than reconstructing it from metadata and memory.
